How long will it take to get a copy of my medical records?
The average turn around time for a request is 5 working days, depending on the completion status and the availability of the record.
How much does it cost?
For patients there may be a copy charge of $5.00 plus .50 cents for each page. There is no charge when you request that we send your information to another physician/health care facility for continuation of care. Attorneys, government agencies and insurance companies should call (910) 678-7244 or 678-7213 for prepayment information.
Who do I tell if my name has changed?
Please mail a copy of the legal name change document (marriage certificate, adoption papers, etc.) along with your date of birth and previous name to:

How long do you retain records?
Charts are kept in active files for 5 years. Inactive charts are stored off site. Medical records are retained for at least 10 years.
I do not want some parts of my medical records released to anyone. What do I do?
Contact the Medical Records Manager at (910) 678-7245 to request that your medical record be flagged to indicate that certain information can be released only with your permission.
Can I take my chart home?
Records can not be removed from the facility. However, you may make an appointment with the Medical Records Manager to review the information in your chart. Please complete the Release of Information form if you would like to receive a copy of your chart. Certain fees and restrictions may apply.
May I look at my family’s medical record?
Unless you are the patient’s legal guardian, you may not review a family member’s record without their signed consent. Please contact the Medical Records Department at (910) 678-7244 for assistance.
Who can look at my record?
SR-AHEC Faculty and staff may look at a patient’s record if they are directly involved in your care or if they need to look at the chart as an approved function of their job. Patient information can not be released from SR-AHEC without a valid patient authorization. |
